Referring to our clinic is very simple.

We ask that you fax the following documents to our HIPAA compliant Fax 980-500-2086.

Once documents are received, we will contact your patient and/or caregivers to schedule an appointment within 14 business days.

Required documents:

  • Patient Demographics and current insurance information.
  • Last office visit note from PCP or Specialists
  • Active Problem List
  • Medication List
  • Recent Labs to include: Vitamin D level, Hemoglobin A1C, Liver Function, CBC w/diff and Basic Metabolic Profile

 

How to Order
Special arrangement have been made with The Tetra Corporation to ensure that you can purchase your prescription from the safety and comfort of your home.
You will pay a flat $7.95 shipping cost to have the medicine delivered to your home.
To use this service simply call Tetra at 800-826-0479, tell them you are my patient and give them the Program Code I gave you.
