Most insurances are now covering telemedicine services, we ask that you contact your insurance for more details related to what they will cover. As always, your co-pay and deductibles will still apply.
New Patients are required to fill out all new intake forms. Once your appointment is confirmed you will receive electronic copies of the forms to fill out.
Established patients, do not have to fill out these forms as they are on file.
A secure web-based portal that allows you to access portions of your chart and communicate with your medical provider.
- Update your health insurance information, upload a copy of your insurance card.
- Update Personal Contact information
- View office visit summaries and clinician letters
- Review your balance due and make payments
- View and print billing states
- Send and receive messages for non-urgent medical advice
- Request prescription refills
- Fill out or upload Patient Forms and Documents
Visits are completed via Facetime, Doxy.me, Client Portal, Telephone Call with photos of condition.
